Form I-693, Report of Immigration Medical Examination and Vaccination Record
Edition Date: 01/20/25. If the civil surgeon signs your form on or before July 2, 2025, you must use the 03/09/23 or the 01/20/25 edition. Starting July 3, 2025, USCIS will accept only the 01/20/25 edition. You can find the edition date at the bottom of the page on the form and instructions.
